Community Voices Video Application

I hereby authorize The Ehlers-Danlos Society to publish my Community Voices segment, recorded by me, or a member of The Ehlers-Danlos Society staff, for use in The Ehlers-Danlos Society’s print, online, and video-based publications.
I hereby release and hold harmless The Ehlers-Danlos Society from any reasonable expectation of privacy or confidentiality associated with the content specified above.
I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any type associated with the taking or publication of the submitted content in Ehlers-Danlos Society marketing materials or other Ehlers-Danlos Society publications. I acknowledge and agree that publication of said content confers no rights of ownership or royalties whatsoever.
I hereby release The Ehlers-Danlos Society, its contractors, its employees, and any third parties involved in the creation or publication of informational or marketing materials, and from liability for any claims by me or any third party in connection with my participation.
