CONSULTS ARE FULLY BOOKED FOR 2024!

Will only be accepting internal referrals from Allina clinicians for 2025 due to high demand and limited availability to 1 new consult weekly. Referrals (e.g. family medicine referral, general referral or other referral stating to be seen for EDS) and documented Beighton score are needed for any consultation including those with a prior diagnosis of Ehlers-Danlos Syndrome. Referral does not guarantee visit due to high demand and limited space. There are age limitations starting at age 5 up to age 45 years old at this time. Referrals must come from medical provider such as a MD, DO, PA or NP. Will not accept referrals from physical therapy, chiropractors, acupuncture, podiatry or other health professionals. Consultations are strictly on Fridays. Dr. Cruz does not see patients with EDS for primary care. EDS/HSD are seen separately and on a different day from her primary care clinic. If there’s a concern for possible EDS, this must be evaluated and ruled out before a patient can establish with her for primary care. Dr. Cruz’s office will reach out to aid with scheduling after referral has been reviewed and accepted. Office staff will reach out to book consults quarterly. Once all consultation times are filled consults will be closed for that 3 month period.
